Position Purpose:
The Nurse Coordinator provides quality nursing care and education to clients and families of the Hemophilia Outreach Center as well as educate the surrounding community on bleeding disorders.
Education/Criteria:
Graduate of accredited nursing program with Associate Degree, or BSN. Nurse Coordinator must be licensed as a Registered Nurse by the State of Wisconsin or reciprocal state. Must have and maintain CPR for healthcare provider certification.

Preferred Knowledge, Skills, and Abilities:
- At least 3 years of general nursing experience required
- At least 1 year clinic experience preferred
- IV infusion experience preferred
- EPIC experience required
Expectations:
Needs to be self-motivated and able to complete work independently. Also needs to work cooperatively with others in a multi-disciplinary team environment. Strong organizational and professional communication skills are necessary.

Patient Care
- Nursing assessments
- Telephone triage of patients
- Treatment planning: writing, implementing, maintaining, updating
- Coordination of Care: acts as a liaison between the Center, Providers, hospital staff, and other outside sources. Communicates and advocates in the best interest of the patient.
- Clinic services: Provides patient care, including, but not limited to medication infusions, bleeding assessments, patient education, etc., on a scheduled and walk-in basis
- Attends Comprehensive Clinics and conducts clinic visits per patient assignments. Coordinates with providers and other ancillary staff to design patient treatment plan
- Provides 24/7 on-call care when clinic is closed 4-5 days every other month with 1-2 holidays per year
- Sends in medication refills as appropriate
- Have the ability to chart all patient encounters thoroughly
- Provide education regarding bleeding disorders to patients and their families as appropriate
Position Type and Expected Hours of Work:
This is a full-time position in a health care facility open Monday through Friday except for holidays recognized by the Hemophilia Outreach Center. Expected hours for this opening are 10 hour shifts, 6:45am until 5:15pm Monday, Wednesday, Thursday, and Friday. Some additional hours are possible on an as needed basis depending on schedule of events, operational demand, and/or patient demand.
